Why Weight Loss Shots Are Working and Still Failing Men Over 40

Why Weight Loss Shots Are Working and Still Failing Men Over 40

Around 40 percent of the weight lost on a weight loss shot comes off as lean mass. Coach Brian works that number in this follower Q&A and pairs it with the habits that decide whether a man holds his result. He is a coach and not a doctor, and the medication decision belongs with your physician.

Weight loss shots work. The men sending Coach Brian questions about them are asking something underneath that, which is why the weight comes back once the pen goes in the drawer. Episode 72 answers four follower questions, and three of them are the reason the fourth one keeps failing.

Coach Brian is a coach and not a doctor. Nothing here is medical advice, and no one should start, stop, or change a prescription based on a podcast episode.

Four Questions With One Answer Underneath

Coach Brian pulled four questions out of his Instagram messages for this episode. How do you bounce back after falling off. What does he think about weight loss shots. How do you set up a calorie deficit when you have no idea what you eat. How do you hit 10,000 steps when your day is packed.

Men send those in as four separate problems. They run on one engine. The medication question is about losing weight. The other three are about keeping it.

What Coach Brian Says About the Shots

His overall read is that weight loss shots are a net positive. They work. They help people lose weight. He has clients using them, and for some men they are the right call.

The mechanism he points to is appetite. The medication turns down hunger. A hard afternoon stops ending in a binge because the pull toward food is weaker.

He also names who stands to gain most from one. Think of a man with a high A1C or a man carrying enough extra weight that his quality of life has narrowed. Coach Brian says the medication can get a man like that back to something closer to a normal life. From there he can go learn the skills that hold it.

The Muscle Number, Checked

Coach Brian cites a study finding that around 40 percent of the weight people lose on these medications comes off as lean mass. On a 20 pound loss that is about 8 pounds. The arithmetic holds.

One caution on how to read the figure. Lean mass on a body scan is not all muscle. It carries water and glycogen and organ tissue alongside muscle, so the muscle share of that loss is smaller than the headline number sounds. Coach Brian makes the wider point himself in the same breath. Any aggressive low calorie approach pulls muscle off a body with or without a prescription attached.

Muscle is the tissue that burns calories. Lose it and the metabolism slows down. His picture for it is a furnace. A bigger furnace burns more fuel. Lose the furnace and the house gets cold fast, and building the furnace back takes a long time.

Working, and Still Failing

Coach Brian's read is that most people use the shots the wrong way. A man takes the medication and eats less and hits his goal weight. Then he comes off it with his relationship to food untouched and his habits unchanged and his day still spent in a chair.

When the medication stops, hunger comes back. He has less muscle than he started with, which leaves him less room to eat before the weight returns. Coach Brian says a large share of people regain most of what they lost.

Losing weight is the easy part. He puts the whole problem in maintenance.

What He Tells a Man Who Asks

For a man considering one, his advice is short. Talk to your doctor. Look at the risks and the rewards for your own situation, and do not do it because everyone around you is doing it.

For a man already on one, he names a requirement. A strength training program that protects muscle. Without that you are losing more muscle than you know, and you feel it later.

He goes further on the training point. A sedentary man loses muscle whether or not a medication is involved, so strength work is on the list either way.

How to Bounce Back Without Going Extreme

Most men mean one bad day when they say they fell off. One meal, one skipped workout, one argument that ended with whatever was in the house. Then one day became two, then a week, then a month.

Coach Brian puts the problem in what happens next. Most men try to compensate by cutting calories low and adding cardio and buying a 7-day detox or a 30-day challenge. His answer on detoxes is that you do not need one. What you need is a plan you never have to recover from.

Going back means going back to the same plan at the same settings. Not a harder version of it.

He keeps one rule for himself. He does not let one bad day become two in a row. A missed workout yesterday makes today's non-negotiable. Food off plan last night means back on plan at breakfast, with no drama attached.

Consistency in his definition is recovery time. The men who hold results are the ones who get back fast. His own target is 80 percent consistency 100 percent of the time.

He also separates fault from response. Not everything that happens to a man is his fault. How he responds to it is his responsibility every time. And he defines failure as quitting rather than as making a mistake.

Calories, and What Men Are Not Counting

The third question came from a man who bulked up, cannot lose his stomach, and has summer coming. Coach Brian's answer is yes, go into a deficit.

He wants men running phases on purpose. There is a gaining phase and a maintenance phase and a fat loss phase. Most men bounce between them at random and get nothing out of any of them. Once a man is in a fat loss phase, Coach Brian likes to see him hold it for 12 to 20 weeks before adjusting.

Then the food. Most men have no idea how much they eat, because the calories arrive in small packages. A Starbucks drink. A handful of nuts. A restaurant salad with everything on it. Bagels and fast food.

High volume foods do the opposite work. Egg whites, Greek yogurt, berries, potatoes, salad with a light dressing and rice cakes. You can eat a lot of that and stay in a deficit without feeling starved.

Then tracking. Weighing your food is the highest return activity Coach Brian names for fat loss. When you know what is on the plate the guessing stops and the scale starts moving. You do not have to track forever, and you do have to track long enough to build real awareness.

His question for the man who resists it is worth sitting with. What are you pretending you do not know about how much you are eating?

10,000 Steps for a Man Who Sits All Day

Someone told him they are too busy for 10,000 steps. He does not accept it, even for a man working 12 hours a day.

He puts the failure in the approach. A man tries to do all 10,000 in one block, takes a 30 minute walk and finds 3,000 on the phone afterward. That feels hopeless, so he stops.

Break it into windows instead. Five minutes in the morning. Ten mid morning. Fifteen at lunch. A short walk after dinner. One more getting to or from the gym.

Then the math he wants men to sit with. Training three or four hours a week is three or four hours out of 168. The workout is not carrying the other 164. Ten or twelve hours a day in a chair undoes a large part of what happened in the gym.

This is why men who train hard and sit constantly are not seeing what they expected. Coach Brian believes in movement 7 days a week. That does not mean lifting 7 days a week. It means the body moves every day, the same way it eats every day.

Activity also drops as men get older. The desk takes more hours. The walks get shorter. He calls it a slow leak in the engine that you do not notice until something breaks.

Your Scoreboard This Week

Five things, and nothing else.

  • Weight trend: weigh in daily and read the 7 day direction rather than the single number
  • Daily steps: 10,000, stacked in short walks across the day
  • Protein: are you hitting your daily target
  • Sleep: 7 hours is the floor
  • One weekly body check: waist measurement or energy or how your clothes fit

Six Things to Do This Week

  1. If you fell off, stop compensating and get back on the exact plan you were on. No detox and no extreme measures.
  2. Do not let one bad day become two. One is fine. Two in a row is a pattern.
  3. If you are on a weight loss shot or considering one, protect your muscle with a strength training program and enough protein.
  4. Track your food for 7 days. Not forever. Long enough to find out what you are eating.
  5. Break your steps into short walks and stack them across the day.
  6. Eat more high volume, lower calorie foods so your plate fills up before your calorie target does.

Frequently Asked Questions

Do weight loss shots like Ozempic work?

Coach Brian's overall read is that they are a net positive and that they work. The mechanism he points to is appetite, because the medication turns down hunger so a hard afternoon stops ending in a binge. He has clients using them and says that for some men they are the right call.

How much of the weight lost on a weight loss shot is muscle?

Coach Brian cites a study finding that around 40 percent of the weight people lose on these medications comes off as lean mass, which works out to about 8 pounds on a 20 pound loss. Read that as lean mass rather than pure muscle, since a body scan counts water and glycogen and organ tissue in the same number. He adds that any aggressive low calorie approach pulls muscle off a body with or without a medication involved.

Why do people regain the weight after stopping a weight loss shot?

Hunger comes back when the medication stops, and by then a man is carrying less muscle than he started with. Less muscle means a slower metabolism and less room to eat before the weight returns. Coach Brian says a large share of people regain most of what they lost.

Should I start a weight loss shot?

That is a conversation for your doctor, not a podcast. Coach Brian's advice is to talk to your physician and weigh the risks and the rewards for your own situation, and to not do it because everyone around you is doing it. He is a coach and not a doctor, and nothing in this episode is medical advice.

What should I do if I am already on a weight loss shot?

Coach Brian names one requirement, which is a strength training program that protects muscle. Without it you are losing more muscle than you know and you feel it later. He adds that a sedentary man loses muscle whether or not a medication is involved, so the strength work belongs on the list either way.

How do I get back on track after a bad day of eating?

Go right back to the same plan you were already on at the same settings, rather than a harder version of it. Coach Brian's own rule is that one bad day does not become two in a row, so a missed workout yesterday makes today's non-negotiable. He treats consistency as recovery time and aims for 80 percent consistency 100 percent of the time.

How do I get into a calorie deficit if I have no idea what I am eating?

Start by weighing your food, which Coach Brian calls the highest return activity for fat loss. Lean on high volume foods such as egg whites, Greek yogurt, berries, potatoes, salad with a light dressing and rice cakes. You do not have to track forever, and you do have to track long enough to build real awareness.

How do I hit 10,000 steps when I am too busy?

Stop trying to do all 10,000 in one block. Coach Brian stacks short walks across the day, starting with five minutes in the morning and ten mid morning and fifteen at lunch and a short walk after dinner. He also points out that training three or four hours a week leaves 164 hours the workout is not covering.
